Health Insurance

Pre-Enrollment Vaccination Form

Student health requirements are specified in the UAMS Academic Affairs policy 1.4.2 Student Health Screening. All vaccination documentation must be uploaded to your GUS account no later than 30 days prior to your first day of classes. You will have a class enrollment hold until documentation is complete.
